Getting a client to sit down and think about exactly what he or she wants from a website is a skill which I expect many good web designers lack. All too often, clients only really start thinking hard about their website requirements once they have been presented with a draft version of their site.

To get clients thinking about their requirements sooner rather than later, I have written a website design questionnaire (PDF, 0.7MB, V2 Jan. 9, 2012). Feel free to adapt this questionnaire and use it with your clients.
